accept all providers when provider is not specified:
This commit fixes the problem when a CAID is set in reader settings,
but the provider is set to zeroes (000000).
Without the patch, a "reader not found" was returned

Fix NDS CWs are dropped when connected to real CCCam servers

fix half cw from mcs

the official Ncam V1.2 r3 doesn't support Constant.cw
you can test this file for ENTV Algerie 7°west and 5°west.
