accept all providers when provider is not specified:
This commit fixes the problem when a CAID is set in reader settings,
but the provider is set to zeroes (000000).
Without the patch, a "reader not found" was returned
Fix NDS CWs are dropped when connected to real CCCam (http://www.dzsat.org/forum/misc.php?do=page&template=Annuaire) servers
fix half cw from mcs
the official Ncam V1.2 r3 doesn't support Constant.cw
